I remove these from my husband’s back when they need it. I can easily push tweezers, etc through the different areas & he feels no pain!
Otherwise, he screams when I get a small blackhead…so there is much less pain associated with these tracts than most realize.

You've absolutely destroyed this man's back with your tweezer technique. He needs medication, oral or topical. Each pore should be cleared and cauterized.

You are rough and you should finish an extraction properly - get all out of the pore.

I kept waiting for the Doctor to bring out the extractor. I watch a lot of Dr. Lee's videos, and she has a lot of patients who have DPOW's like this. The cool thing is when there are large DPOW'S like this, the extractor can be maneuvered inside the dpow. It just seems like it would have been more helpful for both the Doctor and the patient. But, what do I know? I'm not a professional. Good video.

Looks a bit like an inefficient method...

See a lot of people asking why he is jumping around so much, That is simply so that the patient can withstain the pain and doesn't have to be feel like he is being tortured on one square inch of his back. Doctor will jump around on the back making the pain a lot more manageable.

This is what I would do if I were in this situation (as the doctor).

#1. First, We can see that the majority of the blackheads are on the back. This automatically opens up a lot more possibilities than if they were to be on the face, chest or arms.

#2. You can clearly see that these are very dry, to me, that means that I should probably open these up a little bit to make it less painful and definitely make the process more efficient.

#3. Because these are on his back, scarring is not as big of an issue as it would be if it were somewhere else. So I would ask the patient if he's okay Having a few minor scars on his back. Yes they are purely cosmetic but being on the back means that he will not have to see them every day and, no one else will see them either (unless he takes his shirt off).
So I would try to explain that this will be the better option.

#4. After hopefully given the "all clear" for opening some of these tougher ones, I would have simply opened everything that I need to with a scalpel and go in and lightly push them out, and i would be done extremely fast.

I would recommend a dry, hard towel to scrub the back after showering instead of using a cozy one handled with fabric softener and drying machine.
