I work in IT, if it's a bad case fan, find which one it is and trace the fan cable back to the motherboard and unplug it until you can replace the fan. Also it's super easy to replace it, just 4 Phillips head screws and re-route the fan cable.

Also, the reason why the fan noise could be going in and out like that is because the fan speed could be tied to your CPU temperature, and when your CPU temperature goes down so does the fan speed, which could make the fan speed low enough not to make the noise.
