It is impossible to get every single check in one run because well one certain checkmark requires a completely different game mode called greedier and that is separate from the normal run which yes you can get most of them unless you get extremely lucky and get yet another R to redo everything again

I once beat mom, rush bosses, moms heart, hush, Satan, the lamb, mega Satan and delirium in under 2 hours time (single run). If I had restart active item I would have been able to get Isaac and blue baby checkmarks as well. Mother, the beast and greedier were separate runs for me
