A few extra tips for hip assessment in the neonate
- be aware of risk factors in the history eg Family Hx of DDH, breech presentation
- inspection includes visualisation of skin creases looking for asymmetry
- one hip at a time. Be aware how you hold the limb - most cases are picked up by feel, not just visual confirmation
- DDH becomes more obvious the older a child gets. In the UK we do this test as screening both soon after birth, and again at 6-8 weeks. If there is uncertainty or numerous risk factors, child is often seen in a specialist clinic both to rexamine and consideration of USS before child reaches 3 months.

Ortolani's handle which dates back to 1936 frequently fails to detect hip dysplasia. This has been known for over 20 years now.

Ortolani's handle is a test for instability of the hip joint. Instability is only part of the total spectrum of hip dysplasia, and the examination requires a relaxed newborn and an experienced examiner.

Modern present day methods like ultrasound screening and x-ay are far more reliable in detecting hip dysplasia.

There are some points that could have been made and demonstrated that improves the technique: stabilize the pelvis with fingers over spine and thumb over pubis and then examine the opposite limb. In the Ortalani the limb is gently moved with the fingers over the head of the femur so that one can really feel it slip back in over the rim.
