Why I am not receiving emails after filling out google form??

Question: What if the menu doesn't appear when I click on the downloaded add-on? When I click on this add-on (or any add-on, for that matter), it opens an error box with only one option: "Help" When I click that, it just takes me to a forum. Can you help me?

How do i access to “ include form in email”. Its not letting me check it.

Able to add confirmation email. But why are there 2 email inputs in the form? Does respondant have to enter their email twice?
