Dear Jim,  The problem is you! If you are trying to heal Platella Tendonitis, you have to stop all other activities and follow the rehabilitation program. You cannot, I will repeat this for stress, you cannot be continuing with your activities (especially high impact sports such as volley ball, football and tennis) and see effective recovery.
Stop playing tennis, work on your foam rolling, mobility, strengthening of your legs in a controlled eccentric and progressive way and wait for it to recover. If you are going to continue to play tennis, you will not heal and you will only have yourself to blame.

I'm just coming back from a horrible patella tendon rupture. I had surgery to my knee on the 5 of August 2013, it's been 3 months now and last Friday I had a second surgery to take some metal work out of my knee. I'm glad I'm gonna start my rehabilitation program, I'm a very active person who loves sports and gym and in the past 3 months I've been bored shitless lol but I'm glad to be back. Do you have anymore info relating to rehabilitation? Your videos seem to be really helpful, thanks.

I have a chronic case of patellar tendonitis/tendonosis. My physiotherapist told me that the scar tissue in the tendon needs to be broken up before eccentric exercises can be effective. what are your thoughts on this?

Usually if an exercise causes pain then you are advised to stop. However, with these the eccentric decline single leg squat, these often are painful to start with and you are encouraged to work through the pain.

You should continue with your physiotherapist. Single leg decline squats are recommended for patella tendonitis, as well as massage, fructions, stretching and electrotherapies. See our Jumper's knee page on our website for more info

Do you mean that you haven't had physiotherapy since you were 15? If this is the case then you should return and seek more professional help.
