It is not the right top row Book you have to interact with to interact with the Altar, its the row under it. But I first took the poison scrolls out of the top right row before!

As it turns out you have to click the books to interact with the alter.

i cant interact where you place the heart; edit you have to click the top right book before you can interact with the altar

My altar is inactive ,cant click
