I also frequently encounter this issue. My problem is that I can't perform the update via USB stick because a USB hub burnt out. This is the problem, you need two because Sony is a piece of crap and decided that in Safe Mode, you can only connect the controller with a cable, which means I can't plug the USB stick anywhere. Either I'll be lucky at some point and the installation error will disappear, which occasionally happens, or my PS4 is junk. That's why I prefer PC I only have problems with PlayStation.

Thank all for sharing ways to fix it. On my end nothing worked at first so I opened the top. Cleaned up the dust and the bd lens with Qtip dipped in alcohol.

Update still failed.

After reading a post here I tried the safe mode update. (Didn't dind't try after cleaning).... this work. PS4 lives again! FU sony!

Guys, try to get the BD-Ribbon out of the console and clean and check the pins. Clean the with a piece of textile each. Then try to stick the brighter pin (with the bright sticker to the Driver connection and the black one in the motherboard unit. Pay attention that they're holding strong into the pins. This way helped me out outta this shit. Good luck fellas.

If all fails go buy a hard drive for your ps4 to have more space my 500g hard drive couldn’t let me update so I got a 1TB drive and it works help
