There can be two causes to the accident.

(A) the coastal guard plane's pilot did not see the C5 sign.

(B) in an interanaional airport, the coastal guard plane's pilot does not have excellent communication ability in English. In Japan, Coast Guard is to protect the coastline. That means the personnels are navally or militarily trained. Of course, Japan navy and military are trained mainly in Japanese. They use Japanese in their communication mainly in their training and actually practices. For example, China's airforce/army people communicate in Chinese. Russia's army talks in Russian in their practices.
Because of the above, the pilot of JA722A (coast guard plane) did not have good understanding of the instructions from the controlling tower's offcials.

Apparently the dash 8 was told to hold short and clearly didn’t but I still have no idea how the pilots of the a350 failed to see the dash 8 on the runway

I remembered there was an accident, similar due to the WRONG words being used
and this was
"clear to taxi short of takeoff"
which was changed to
"clear to taxi short of runway"

do you recall which was that accident?

I believe there was a communication error between those involved. By international agreement English is used for air traffic communication. But English is a second language to these pilots and ATC. One simple miscomprehension could easily lead to an accident.

The coast guard plane was the only one saved and repaired in the big earthquake years ago, and was a symbol of hope. Now it has become a terrible omen, 2024 is scary already.

I don't know full procedures but having a single point of failure seems poor. IE pilot being told to hold short but continued onto active runway by mistake. The should have some type of barrier or traffic light system in place for this ? Live and learn. Feel bad for ATC and pilot here.

I cannot understand why the planes were on different frequencies. Or was the Dash Eight still on the ground control frequency and erroneously taxied into the runway?
Could be that the Coast Guard crew was not familiar with night operations in such a large airport? I assume that they normally fly on some smaller military airfield.

There are multiple interviews of the passengers indicate several systems on the A350 failed. Intercom was down, door system was down, evac light or some lighting was down, so it was very chaotic. The crew has to manually open the doors. With multiple failures, the safe evac is mostly attributed to excellent crew training and the discipline of Japanese people. If this was in the US, China, or India, I bet there would be more fatalities.

The Dash 8-300 is CLEARLY standing still. Either the pilots of Japan Airlines didn't see it in THE MIDDLE of the runway, or the Japan Airlines plane landed OUTSIDE and to the left of the runway.

Cant there be physical barriers that go up to prevent airplane from going into the live run way?

Like cant we pay people minimum wage to wave a flag there when runway is LIVE or something? jesus christttttt.

First of all, condolences for all the fatalities and their relatives I wish great strengths in their tragic loss. It is always hard when a relative passes. For those who didn't know them the mere fact of them being helpers of an even greater tragedy (the earthquake) should make to feel piety… RIP!
And big BIG kudos to the JAL flight crew and also to the passengers! They all shown that Japanese traditions and way of attitude are still high and venerable.
Finally, big thank you, Captain Dennys! Your commentary was really insightful. Before this I was Juan Brown's explanations and I had a slight doubt whether I will hear or see something new, but yes, I did! Your points were perfectly clicked into Blancolirio's and the one plus one finally added up to three!

ATC Transcript: Coastguard aircraft was told to "taxi to holding point C5". (The Captain had told investigators he had been given permission to enter the runway.)
EDIT Source: BBC News. Not sure how the Captain was able to talk with invesigators if he was medically in a critical state.

If the captain of the Dash 8 has a concussion, he may not remember anything about the events leading up to the collision.
Another interesting fact people may not know. Airports need the first fire truck to be able to reach anyplace on the airfield within 90 seconds of an alarm. Sometimes they'll have two fire stations on big airfields to be able to meet this requirement. They do drills often to prove they can accomplish this.

It’s clear all pilots and all tower controllers had a part in this crash .
Lots of blame to go around if any of them had been on the ball doing what they are supposed to be doing this would not have happened .
Accidents like this are a result of a chain of events not just one bad decision

The JAL plane was burned to the ground. Most or all structures of the airbus are composition materials. This always in my ear. Yes the materials are very light, not heavy, but I am questioning the flammability of the materials. How strong is the material withstanding fire on its skin, fuselage before the fire come into its inside.

The NTSB made an urgent request for action to be taken to completely avoid runway incursions following an accident.
That was in 1969.
They make the same Urgent Request every year.

Pilots remember before entering an active, say, lights, camera, action. Lights (all required exterior lights on) camera ( Take a mental view of the runway and app end clear left and clear right.) action (set transponder, etc.). and again say clear left and clear right.

Such a shame for those lost lives on the coastguard plane. I have a horrible feeling the outcome for the A350 would have been so very different if this happened in most other countries. Not one of those disciplined Japanese passengers took their cabin luggage. Crucially, it also looks like the fire hadn't breached the cabin by the time the last passengers were escaping.

Airbus is on its ground roll. Clouds shouldn't matter, I would think.

One thing not to ignore is that the Flight Attendants evacuated all of their passengers without any losses. That is both Professional and Heroic to me. Guess who was the last to leave the smoke filled cabin.

It's ironic that the person that most likely caused this accident is the only one that survived on the Coast Guard aircraft--the captain. The captain is the one that taxis the aircraft and the Coast Guard aircraft shouldn't have been on the runway.

Thank you very much for picking this horrible accident up! It´s indeed a kind of a miracle that all people on board of the A 350 survived - and there´re indeed a lot of questions open and to be answered why this could happen.
