Adding Multiple Google Forms to One Spreadsheet

I can't find this form when bring chenge setting

Is this still working? I tried and the sidebar does not load. This could have been perfect

Does this make copies/duplicates of the same form? Or does it generate brand new forms?

I really like this. On another note, how do you get the magnifying glass when presenting?

Google completely blocked this. No way to authorize...:-(
