DVWA - file inclusion low, medium and high

In medium level we can make LFI just with /etc/passwd as input, since ../../ is filtered

How can I upload the files?
, I cannot open the remote file why?

in dvwa v1.8 high level has only one filter i.e.. allowing only include.php ,so one can no more use "file" and proceed for local file inclusion.
