With my very first character ever when the game first came out I actually somehow ended up doing all of their quest lines in the first playthrough and I got the chaos flame and was at the elden beast but never got to beat it but I'm playing it again now

The "delusion" she is referencing I believe is love. She, due to the Two Fingers and Order, has become a somewhat callous being, shedding away mortal comforts and acting in her own interests ruthlessly to ensure the death of her Two Fingers.

However, there are glimpses of a formerly kind and caring soul in her quest. Despite being stern and cold, after slaying the Shadow, her tone changes to tell you to tell her friends that she loves them. And again, here at the end, she genuinely believes to have found a kindred spirit in you, to travel the stars together. But when you betray her, she sheds yet another layer of emotion, abandoning the concept of love to be a mere delusion. Additionally, she seemingly didn't need you and may take off to the stars by herself here, meaning she truly did believe that you were a soulmate of a kind.

I don't remember Ranni showing up like that. I didn't pledge to be her eternal simp, but I had the option for her ending in the Erdtree...weird.
